I designed this tag using Amy Marie's "four of 2014" scrapkit.

Tutorial
*Important note dropshadow is used on all elements*
Open new 500x500 page.
Place element 20 in the middle of the page.
Resize to X 256.52px, Y 250.15px, W and H 350.55%.
Place paper 11 behind the element 20.
Erase any parts of the paper outside of the element.
Place element 43 behind the paper.
Resize to X 257.50px, Y 268.00px,W and H 100%.
Place element 126 at the top of element 20.
Resize to X 258.20px, Y 80.99px, W 449.52% and H 411.69%.
Place element 111 at bottom left hand corner of element 20.
Resize to X 128.00px, Y 389.45px, W and H 175.12%.
Place element 119 infront of element 111.
Resize to X 137.88px, Y 463.31px, W and H 145.58%.
Place element 51 at the right hand side of element 119.
Resize to X 208.10pz, Y 463.11px, W - 74.76% and H 74.76%.
Place element 128 at left hand side of element 119.
Resize to X 67.19px, Y 466.65px, W and H 81.78% and -5.06 degree angle.
Place element 110 at bottom right hand side of element 20.
Resize to X 353.50px, Y 446.00px, W and H 145.83%.
Last but not least do not forget tube license and give credit to scrapkit maker.
